All applications shall be filled out completely.

All applications shall be accompanied by a copy of applicants current license, insurance certificate and workers compensation affidavit.

All service changes, siding permits or any work that requires meters to be resealed a Utility Authorization Number shall be obtained and be written on permit.

Be advised that permit fees will be reviewed by the Electrical office and any discrepancy in fees will need to be paid prior to permit being issued.

No permits will be issued without all proper information supplied with applications.
